Congratulations on your success at interview for FitFutures.

To ensure we can proceed with your application and get you started, you will need to complete the short enrolment form below as part of our duty to ensuring we can accommodate your learning needs and get you officially registered onto the qualification.

Once this form has been completed, you will be taken to a separate page to confirm, if relevant, how you will be financing the other 50% of the fees for your training.

*Please note, for those of you on the 100% fee work back, please simply close the student finance page.

If 'No', you will not be eligible for funding.
Please indicate the Subject, Level of Study, Grade and date achieved. If none, put 'None' in the box.
We are required to obtain this information to monitor our equality and diversity policy.

By clicking submit you are confirming your space on the agreed training programme. A space will be confirmed for you by both the employer and us as the training provider.

By clicking submit, you are confirming your enrolment and commitment to the programme. Should you withdraw from the programme after it has commenced, you will have incurred fees for the training booked and/or delivered due to the financial investment your employer has made into you and your training. Should you withdraw, you MUST give us notice of at least one month to avoid incurring any fees.
